Accurately interpreting medical records can be difficult and time consuming.  The key facts are sometimes obvious but often there are implications and presumptions hidden between the lines.  Was there adequate evidence for that doctor’s diagnosis?  What was he hesitant about before choosing therapy?  Were all the risks considered and discussed?  Were alternatives considered?   What was unusual about the patient's presentation?  What outcome was initially expected?  What changed?  And is there anything important on all those handwritten pages?
