The Agreement defines the services offered by MMC and the legal obligations of MMC and each Client (generally the firm or lead attorney in the case).  An Agreement must be completed and signed by the Client prior to review of records for a project.

To complete the Agreement, fill in the fields with the firm's information including a named representative, as well as the fields on the final page with the case title and other options.

Please sign the form once printed.

The document should be mailed to MMC along with records, or sent via fax (please call for the number).   A copy signed by Dr. Merrill will be promptly returned to the client.
